Questions to Ask Your Insurance Company about Coverage for Lung Cancer Screening

Lung cancer screening means testing for lung cancer before you have any symptoms, such as a chronic cough or shortness of breath. For those who meet the high-risk criteria, screening is typically covered by Medicare and most private insurance plans with no cost sharing. However, before you schedule your low-dose CT (LDCT) scan , it is important to know exactly what your insurance covers. Use the questions below as a guide for your conversations with your insurance company.

On the back of your insurance card, there will be a phone number for you to call. You will likely be asked for your plan information and group ID.

  • Does my insurance company have a website or a flyer about lung cancer screening coverage issues?
  • Do I need to see my primary care provider or doctor first to get a referral or to confirm if I am eligible for screening?
  • What forms or paperwork do I need to complete before, during or after that visit to make sure my screening is covered by insurance?
  • Do I need a referral or preauthorization for a LDCT?
    • If so, who provides it?
    • Who should this referral or preauthorization be sent to?
    • Will my physician submit it, or do I need to?
  • Is there a specific facility or center I need to go to for my screening?
    • If the nearest facility is too far away from where I live, what do I do?
  • Will my insurance cover my travel expenses to and from the screening facility?
  • Is the screening fully covered, or will I be charged with any out-of-pocket expenses?
    • Will I need to pay a facility fee?
    • Are there other out-of-pocket costs I will need to pay?
  • Who do I follow up with after my screening?
  • Are follow-up doctor visits covered by my insurance?
    • If not, what out-of-pocket expenses should I expect?
  • If something is detected on my LDCT scan, will follow-up tests be covered by my insurance? 
  • Is comprehensive biomarker testing covered by my insurance?
  • If my doctor recommends annual screenings, will my insurance cover them?
  • Will my insurance cover counseling and medications that can help me quit smoking? What out-of-pocket expenses should I expect?

Tip: Take notes during your call with your insurance company.  Write down the representative's name and the date of your call. This can help if you need to confirm your coverage later.
