Knowing if you have ALK-positive lung cancer has important treatment implications, particularly for people with advanced-stage disease. Many patients with advanced-stage ALK-positive lung cancer will receive an ALK inhibitor, also known as a tyrosine kinase inhibitor (TKI), during their treatment journey.
ALK inhibitors target changes in the ALK gene that help cancer grow and spread.

ALK inhibitors work to stop or inhibit cancer growth by targeting the error in the ALK gene.
Treatment Based on Stage of Disease
When Treatment Stops Working
Over time, ALK-positive lung cancer may stop responding to your current ALK inhibitor. When this happens, your doctor will discuss the next steps for treatment, which may include one, or a combination, of the following options:
- Another ALK inhibitor
- A re-biopsy to determine if a specific ALK-resistance mutation or other changes have occurred
- Increasing the dose of your current ALK inhibitor
- Traditional chemotherapy
- Immunotherapy
- Bevacizumab (Avastin), another type of targeted drug that starves the tumor
- Radiation therapy
- A medicine being tested in a clinical trial
Additional biomarker testing through a tissue or liquid biopsy may help identify changes that can inform future treatment decisions.
Brain Metastases
Sometimes, ALK-positive lung cancer may spread to the brain, called brain metastasis. In that case, your doctor will recommend an ALK inhibitor that can address the cancer cells in the brain. Some patients may need surgery and/or radiation.

Research in ALK-positive lung cancer continues to advance rapidly. Clinical trials are evaluating new therapies and approaches to help improve outcomes for patients with ALK-positive lung cancer.
