ALK-Positive Lung Cancer Treatment Options

Learn about treatment options for ALK-positive lung cancer, including ALK inhibitors, treatment after progression, clinical trials, and questions to discuss with your care team.

Knowing if you have ALK-positive lung cancer has important treatment implications, particularly for people with advanced-stage disease. Many patients with advanced-stage ALK-positive lung cancer will receive an ALK inhibitor, also known as a tyrosine kinase inhibitor (TKI), during their treatment journey.

ALK inhibitors target changes in the ALK gene that help cancer grow and spread.

ALK Inhibitors 

ALK inhibitors work to stop or inhibit cancer growth by targeting the error in the ALK gene.

  • Alectinib (Alecensa) 
  • Brigatinib (Alunbrig) 
  • Ensartinib (Ensacove, Xcovery) 
  • Lorlatinib (Lorbrena) 
  • Crizotinib (Xalkori) 
  • Ceritinib (Zykadia)

Treatment Based on Stage of Disease

Some patients with early-stage ALK-positive lung cancer who have had surgery may receive an ALK-inhibitor called alectinib (Alecensa). It can be given with or without chemotherapy

If you had surgery and your tumor was ALK-positive, ask your oncologist whether adjuvant alectinib (Alcensa) might be appropriate. Make sure ALK status was confirmed using an FDA-approved test, and ask about the expected benefits and side effects for your specific cancer stage. 

Patients with advanced-stage ALK-positive lung cancer will likely receive an ALK inhibitor at some point during treatment. Your care team will recommend treatment based on your individual situation, treatment history, and overall health.

When Treatment Stops Working

Over time, ALK-positive lung cancer may stop responding to your current ALK inhibitor. When this happens, your doctor will discuss the next steps for treatment, which may include one, or a combination, of the following options:

  • Another ALK inhibitor
  • A re-biopsy to determine if a specific ALK-resistance mutation or other changes have occurred 
  • Increasing the dose of your current ALK inhibitor
  • Traditional chemotherapy
  • Immunotherapy
  • Bevacizumab (Avastin), another type of targeted drug that starves the tumor
  • Radiation therapy
  • A medicine being tested in a clinical trial

Additional biomarker testing through a tissue or liquid biopsy may help identify changes that can inform future treatment decisions.

Brain Metastases

Sometimes, ALK-positive lung cancer may spread to the brain, called brain metastasis. In that case, your doctor will recommend an ALK inhibitor that can address the cancer cells in the brain. Some patients may need surgery and/or radiation.

Clinical Trials

Research in ALK-positive lung cancer continues to advance rapidly. Clinical trials are evaluating new therapies and approaches to help improve outcomes for patients with ALK-positive lung cancer.

Ask your doctor whether a clinical trial may be appropriate for you.

Questions to Ask Your Doctor

Work with your doctor to discuss your goals and options each time you have to review and interpret test results to make a treatment decision. The three big questions to ask are:

  • What is the goal of this treatment?
  • What are the potential side effects?
  • What other options do I have?

Research is happening at a rapid pace, and your doctor should be up to date on the recommendations for your specific type of lung cancer. If you don’t feel comfortable with the answers you are receiving, do not hesitate to seek out a second opinion.
